[DCCP] ccid3: Make debug output consistent

This patch does not alter any algorithm, just the debug message format:

 * s#%s, sk=%p#%s(%p)#g

 * when a statename is present, it now uses %s(%p, state=%s)

 * when only function entry is debugged, it adds an `- entry'
