Remove GrRefCnt.h in favor of SkRefCnt.h

This removes GrRefCnt.h with all its tyepdefs and #defines and just switch them
to the Sk* equivalents.

GrSafeSetNull was promoted to SkSafeSetNull in SkRefCnt.h.

TEST=none, no functional changes.
