Refactor the way PCHReader tracks whether we are in recursive loading. -Replace CurrentlyLoadingTypeOrDecl with a counting scheme (NumCurrentElementsDeserializing) -Provide outside access to the mechanism by adding methods StartedDeserializing/FinishedDeserializing to ExternalASTSource.
