Small cleanup: * Remove some unused (mostly empty) functions * Added context parameter to WAIT_IDLE_EMPTY[_LOCKED] for consistency * Added debug messages to WAIT_IDLE_EMPTY[_LOCKED] * Don't flush empty command buffers
