block, drivers, fs: rename REQ_FLUSH to REQ_PREFLUSH

request_fn drivers, and upper layers requesting the block layer
perform a flush sequence along with possibly a WRITE, this patch
renames REQ_FLUSH to REQ_PREFLUSH.
