memcg: rename MEM_CGROUP_STAT_SWAPOUT as MEM_CGROUP_STAT_SWAP

MEM_CGROUP_STAT_SWAPOUT represents the usage of swap rather than
the number of swap-out events. Rename it to be MEM_CGROUP_STAT_SWAP.
