[PATCH] timer initialization cleanup: DEFINE_TIMER

Clean up timer initialization by introducing DEFINE_TIMER a'la
DEFINE_SPINLOCK.  Build and boot-tested on x86.  A similar patch has been
been in the -RT tree for some time.
