[PATCH] pipe.c/fifo.c code cleanups

more code cleanups after the macro conversion:

 - standardize on 'struct pipe_inode_info *pipe' variable names
 - introduce 'pipe' temporaries to reduce mass inode->i_pipe dereferencing
