Many files:
  debugfs.c, debugfs.h, dump.c, icheck.c, ls.c, lsdel.c, ncheck.c,
  	setsuper.c, util.c: Change ino_t to ext2_ino_t.  Fix a few minor
  	gcc-wall complaints while we're at it.
