Split fpdfsdk/fsdk_baseform.h into individual classes.

This CL moves classes in fsdk_baseform.h to their own files.
Classes include CPDFSDK_Widget, CBA_AnnotIterator, CPDFSDK_XFAWidget,
PDFSDK_FieldAction, and CPDFSDK_Interform.
