Re-land "Stack profiles: Reuse existing frame/module rows rather than adding one per sequence"

This reverts commit 708d796a85a1cf77b16bb8877dcfce0850c9b268.

Re-landing with fixes to deal with multiple frames with the same mapping_id+rel_pc.
