Remove all \bcode / \ecode cruft; this is no longer needed. See previous
checkin of myformat.sty.
Change "\renewcommand{\indexsubitem}{(...)}" to "\setindexsubitem{(...)}"
everywhere.
Some other minor nits that I happened to come across.
