PBAP: Use ACTION_CONNECTION_STATE_CHANGED intent

* Use ACTION_CONNECTION_STATE_CHANGED to broadcast connection state
  change to comply with BluetoothProfile interface requirement
* Use BluetoothProfile.STATE_* variables to represent connection
  states for PBAP profile
