[codeview] Describe int local variables using .cv_def_range

Summary:
Refactor common value, scope, and label tracking logic out of DwarfDebug
into a common base class called DebugHandlerBase.

Update an old LLVM IR test case to avoid an assertion in LexicalScopes.
