1903 MaxAuthTries=0 a useful way to block users entirely (esp. in a
1904 sshd_config Match block).
1905 Also, move sending of any preauth banner from "none" auth method
1906 to the first call to input_userauth_request(), so worms that skip
1907 the "none" method get to see it too.

2462 on error paths, and make it fail gracefully on out-of-fd conditions -
